Mass General Brigham Health Plan to offer Dual Eligible Special Needs Plans in 2026

October 9, 2025 (SOMERVILLE, MA) – Mass General Brigham Health Plan today announced that, beginning January 1, 2026, it will offer two Dual Eligible Special Needs Plans (D-SNP) for individuals eligible for both Medicare and Medicaid. 

 

The Executive Office of Health and Human Services (EOHHS) approved Mass General Brigham Health Plan to provide dual-eligible coverage through the state’s One Care and Senior Care Options (SCO) programs. One Care mainly serves disabled adults aged 21 to 64 who require extensive community-based supports, focusing on recovery, behavioral health, and independent living. SCO mainly serves adults aged 65 or older, some of whom are disabled, emphasizing aging in place and chronic condition management through the coordination of long-term services and support.

Dual-eligible individuals face a variety of barriers to accessing care, from coordination of complex medical support to needs for daily living, such as transportation. Furthermore, these individuals may not be aware of their eligibility for healthcare coverage through a managed care plan. Approximately 33% of Massachusetts residents eligible for both Medicare and Medicaid were enrolled in a D-SNP plan as of 2023, according to EOHHS data

As part of an integrated healthcare system, Mass General Brigham Health Plan is working to expand education about eligibility and opportunities to access more coordinated care and support. This work also includes building integrated care models with the system’s population health team to extend the reach of primary care providers beyond the clinical setting through personalized care plans and extensive care management resources.

Through Mass General Brigham Health Plan’s D-SNP coverage, enrollees can access a regional network of doctors and hospitals and extensive social supports. This includes Dukes and Nantucket counties, where D-SNP plans will be offered for the first time to eligible residents to expand care and coverage options.

One Care and SCO plans by Mass General Brigham Health Plan will be available for selection during the Annual Enrollment Period from October 15–December 7, 2025.
